Especially if the game makes modifications to your choices/settings. I wanted to raise the issue before but it felt self serving when I was seemingly the only one impacted.

There are an insane number of day to day injuries. It's incredible.
In an online league that ran 18 for my first time last season, I had 6+ players with day to day injuries that ran for weeks. My catcher struggled with a day to day hamstring injury for 3 months. It was frustrating but fun.
You can place guys on the DL and have them recover faster but it's no guarantee. The game only says "unknown" when telling you the recovery length. So you can place your star player on the DL for two weeks and see no change.
I do think 18 is a much better game just for how it handles injuries. It's a small feature that really has a huge impact on the game.
That's just about the only difference I think we would see. We can always wait for 19 or whatever.
